Industrial accidents are part of life (unfortunately)...
For each entity, be able to assign maintenance allocation... 100%, 90%... to be able to save on maintenance cost at the expense of accident/breakdown risk.
Assign RISK parameter to each entity... with modifier if maintenance is neglected.
Example: Oil refinery, if 100% maintenance budget, accident risk is 1/1e6 CAT, 1/1e4 MAJ, 1/1e3, MIN... have it explode... wrecking the unit and surrounding items, as well as kill x amount of workers depending on severity.
