I started playing for the first time in a bout two months and this started happening on the same game I've been playing that whole time.
They disappear after some period of time, my best guess is there is some lag\breakdown in the system that says what to render.
It seems to be pretty rare when it happens, the vast majority of the icons in areas being actively mined work normally.
They should update when your selection of which bars to show changes.
Occasionally, some bars get stuck, but turning the layers view off completely and on again fixes that.

no, I only enable them for short periods of time. max 30 seconds at a time fairly regularly I don't use the tool very often
