I found no train limits worked for trains with multiple inputs, as everytime it did happen the station was slot capped.
Trains get removed from request schedule due to bug

mostly agree. for some reason the train gets removed from the requesting station schedule, another train gets inserted, and the bugged train gets "lost"
This only occurs with stations that have an enable condition, right? A station always accepting trains never shows, right?

That's actually a very interesting data point, read this
(this used to be true at least): https://discord.com/channels/803508556325584926/803519296864518184/1384594353053503539

it only ever happens at this specific supply station
I have like 7 other rock supply stations all feeding rock demand

so its something unique about this specific station
and I've tried demolishing and rebuilding the station completely, no dice
see picture, yes
I have 30 supply stations for all raw ores running all modules full
and this is the only station where the bug appears
Seems the slot cap is capible of stopping a train in its tracks, but not arrival conditions.

TLDR: the train distribution logic changes (or at least used to),
based on wether or not one had train limits set in any station
with-in a train "line"'s "stop" (a.k.a. station group).

if you set a request threshold higher t han the supply at the station, the train should still reach the station and wait there until it is full and can depart

this works great and helps increase throughput on large train networks
If you remove limits the station will request ALL the trains when the station is empty, I have that issue with my 16 water trains.
I now build things that need water near water
It may not work well, but try removing the request train only if, and limit to 1 train (2 if you have stacker space) based on all the discussion it seems like the problem may be that during recalc the train station fails that condition and a bug removes the train from pathing
well just deleting the station and running off of the other one I've built next to it is the ultimate solution

Yup, that's what I'm talking about.
If there are no limits anywhere, then trains
just get assigned to one station, as there's nothing
stopping them from picking just that one.
If there is at least one on some station,
the trains start getting allocated evenly.
But to the point, maybe the code that handles this
is responsible, that is my point.
but I'd like to supply the devs with as much information as possible
Trains will get a upgrade in U4, hoping for some upgrades in train handling.
But for now the shipping station group/recieving station group seems to be the way to go. This problem will persist with that.
well again none of my other stations present the bug - there's been a rock supply on the quartz mine running 3 request for like 1000 years
hasn't presented an issue there a single time
so for whatever reason it's this specific location
idk why
maybe the distance does come into play
as there are further distance rock mines, but they are low throughput

So will this save reproduce the error if I run it forward, or is it in an already broken state?
I can't get it to be predictable I'm sorry. I've been unsuccessful at an auto save replicating the issue which happened
Anytime I load them up, it just doesn't happen
